Verification of benefits is not a payment guarantee
A verification of benefits is a snapshot of plan language on one call: deductible left, coinsurance, and whether residential care is a listed benefit. It is not a promise the claim will pay. Authorization, medical-necessity review, and network status still sit downstream. This page is a decoder. It is not a billing desk.

Get the plan words, not only the campus note
- Call the number on the card. Write the reference number and the agent name.
- Ask whether the check used the sleep-address NPI and the exact level of care.
- Ask what still has to happen before a day is payable (prior auth, concurrent review).
Script: insurer call. Network language: in vs out of network. Calendar: prior-auth calendar.
Phrases that are not a guarantee
- We take your insurance.
- Benefits look good.
- You are covered for 30 days.
Those sentences skip medical necessity and the last authorized day. See also red-flag glossary.
