In-network vs out-of-network residential rehab
In-network means the insurer and the licensed program have a written contract for a listed level of care. Out-of-network means they do not, even if admissions will take the card and file a claim. Confirm the sleep-address NPI on the member line printed on the card. This page is a decoder. It is not a benefits check and not a placement service.

What to ask the plan, not only admissions
- Is this NPI in-network for residential (ASAM Level 3.x), for detox, and for PHP or IOP after the stay?
- What is the allowed daily amount in-network versus out-of-network?
- Separate deductible or out-of-pocket maximum for behavioral health?
- May the program balance-bill above the allowed amount if it is out of network?
Use the insurer call script. A verbal we-take-your-plan from a salesperson is not the contract.
Why the two answers diverge
Admissions may mean they will bill. The plan means they have a contracted rate. Out-of-network residential can still pay something after deductible, or it can pay nothing until a single-case agreement exists. See single-case agreement and verification is not a guarantee.
