After the stay
The first outpatient appointment after residential
Leave campus with a dated next appointment and a phone that belongs to that clinic. A promise that “someone will call you” is not a slot. Ask for the date before travel home, write it on the aftercare plan, and take medicines in original bottles to that visit. This page is a question list. It is not a clinic and not a placement service.

What to write down
- Clinic name, street address, date, time, and facility-owned phone.
- Level of that visit: office, IOP, or PHP. See step-down.
- What to bring: ID, insurance card, the medicine list. See medicines at discharge.
- Who may call if the person misses the slot, if a release is on file. See HIPAA and family authorization.
If the date slips
Call the clinic on the paper, not a marketer. If danger appears before the visit, use 911 or 988. See relapse after discharge.
