Discharge
Discharge-day questions for a residential stay
Discharge day is logistics plus paper. Confirm the pickup window at the sleep address, what leaves in the bag, and the dated next appointment before anyone is in the car. Use the facility-owned number you confirmed at admission. This page is a question list. It is not a transport desk and not a placement service.

Questions for the campus number
- What time is the pickup window, and which door at the sleep address?
- Are ID, insurance card, phone, and original-bottle medicines in the bag?
- Is there a written discharge summary or aftercare sheet to take home? See aftercare plan in writing.
- Who writes the next prescription, and how many days of medicine leave campus? See medicines at discharge.
- Will staff speak with family at all, or only with a signed release?
If the day is not the planned day
If the person is in danger now, call 911. Mental-health crisis: 988. An early walk-out is a different page: when someone leaves AMA. This site does not send a car and does not write a commitment petition.
Travel home
If the road itself is not safe — withdrawal on a flight, no ride, a storm — stop and use 911. Arrival questions on the way in were a different list: travel-day questions.
What this page does not do
It does not book a flight or a sober-living bed. Confirm logistics with the licensed campus. Look programs up on FindTreatment.gov.
