After the stay
A continuum-of-care contract is not a second license
A continuum-of-care contract is a written package that names more than one level after the residential bed. It can be useful. It is still a commercial promise. It is not a second state license, not an ASAM assessment, and not a guarantee that the next address is licensed for the care it sells. This page is a question list. It is not a clinic and not a placement service.

What to put on the paper
- Each level by name: residential, PHP, IOP, office — see ASAM levels and step-down.
- The street address where that level actually runs, and the license that sits at that address. See license lookup.
- What is housing versus treatment. A sober-living lease is not automatically a clinical hour.
- Price, refund, and what happens if the person declines the next level.
What the contract is not
It is not a substitute for a dated aftercare plan. It is not FindTreatment.gov. Confirm each site on FindTreatment.gov and the state directory.
