Discharge
Medicines at discharge from inpatient rehab
Ask what leaves in the bag: a written list, original bottles when the campus issues them, and the name of the clinician who writes the next refill. A verbal list in the parking lot is not a handoff. This page is a question list. It is not a pharmacy and not medical advice.

Questions for the campus number
- What medicines leave today, in which bottles, and for how many days?
- Who writes the refill, and is that appointment dated on the aftercare sheet?
- Were any home medicines held at admission returned?
- If family is driving, who is allowed to carry controlled medicines?
Pair this list with discharge-day questions and the written aftercare plan. What to pack on the way in: packing list.
If something is missing
Call the facility-owned number before you leave the drive. This site will not fill a gap and will not name a pharmacy. If the person is in danger now — withdrawal, overdose, a medical collapse — call 911.
What this page does not do
It does not recommend doses or brands. It does not operate a refill desk. Confirm the handoff with the licensed campus.
